The investigative committee chaired by Acting Minister for Defence Dr Cassiel Ato Forson has submitted its final report on the tragic military helicopter crash that claimed eight lives on August 6, 2025.
The presentation took place on Monday, November 10, during a National Security Council meeting at the Jubilee House, Accra, presided over by President John Dramani Mahama.
The committee, established shortly after the fatal incident, was tasked with determining the cause of the crash and proposing preventive measures to avert similar tragedies in the future.

According to government sources, the comprehensive report delves into technical, operational, and environmental factors that may have contributed to the accident. It also provides a series of recommendations aimed at strengthening aviation safety protocols within the Ghana Armed Forces and national emergency response systems.

The findings of the report are expected to be made public on Tuesday, November 11, 2025, at an official press briefing. Officials are anticipated to outline key conclusions, remedial steps, and possible reforms within the Air Force’s operational framework.
The fatal crash involved a Harbin Z-9EH military helicopter (tail number GHF 631) operated by the Ghana Air Force. The aircraft departed from Accra around 9:12 a.m. on a mission to Obuasi in the Ashanti Region as part of an anti-illegal mining operation when it lost radar contact and crashed in the Adansi Akrofuom District.
Tragically, all eight passengers and crew members aboard perished. The victims included:
- Dr. Edward Omane Boamah, Minister for Defence
- Ibrahim Murtala Muhammed, Minister for Environment, Science and Technology
- Muniru Mohammed Limuna, Acting Deputy National Security Coordinator
- Samuel Sarpong, NDC Vice-Chair
- Samuel Aboagye, former parliamentary candidate
- Sqn. Leader Peter Bafemi Anala
- Flying Officer Manaen Twum Ampadu
- Sergeant Ernest Addo Mensah
The investigation, overseen by National Security Coordinator Abdul-Osman Razak, was conducted in collaboration with the Ghana Air Force, the Ghana Civil Aviation Authority (GCAA), and international aviation safety experts.
